While Original Medicare Part A provides important hospital coverage, there are real gaps—and those gaps can add up quickly.
Here’s How It Works in 2026
Each benefit period begins with an inpatient hospital deductible of $1,736.
After that, your share of the costs depends on how long you stay:
Days 1–60:$0 per day after the deductible is met
Days 61–90:$434 per day
Days 91–150:$868 per day using your 60 Lifetime Reserve Days
Lifetime Reserve Days Don’t Reset
For each stay longer than 90 days Medicare gives you 60 Lifetime Reserve Days—not per year, but for your entire lifetime. Once they’re used, they’re gone for good. And if a hospital stay goes beyond 150 days, Original Medicare stops paying entirely.

A 90-day hospital stay could be around $15,000 in out-of-pocket costs
A 150-day stay could exceed $67,000
There’s No Cap on Your Costs
Original Medicare does not include an out-of-pocket maximum for hospital care. That means there’s no built-in limit to what you could spend during a serious or extended illness. It’s so important to have a plan in place to complete your Medicare coverage.
